Training and Technical Assistance
PACT is continually looking for ways to improve health systems’ capacity to serve its most vulnerable, often most costly, patients. This is becoming especially relevant as more healthcare systems are considering patient-centered medical home and accountable care transformations and many payers (private, public, and foundation) have developed an interest in the cost-saving benefits of these transformations.
To date, PACT has provided training and technical assistance support to multiple organizations, including New York City’s Department of Health and Mental Hygiene (assisting 28 Ryan White funded clinics improve the care of their HIV/AIDS patients), the University of Miami School of Medicine, and the Navajo Nation in New Mexico.
PACT has also successfully integrated CHWs into a community health center; Codman Square Health Center in Dorchester, MA sought our assistance to train and integrate CHWs to work with their most high-risk diabetic patients. In 2010, PACT worked directly with Network Health (a Massachusetts Medicaid managed care organization), Commonwealth Care Alliance, and Cambridge Health Alliance to bring a CHW- and NP-driven, wrap-around care management platform to their most costly and most complex patients.
PACT desires to continue to innovate with new forward-thinking and socially conscious partners. We strongly believe that well-supported CHWs and a focus on quality improvement can enable a health system to better accompany its patients to better health, better quality of life, and better utilization of the system.
