In Alabama, we’re collaborating with public sector and community-based organizations to strengthen the community-based workforce and remove barriers to mental health care.

Serving as a strategic partner to the health department, we’re helping to drive more coordinated health equity programs and increase community engagement and collaboration with Tribal Nations.

We are advising partners in Southeast Florida on the design and management of community health worker programs, including equipping community health workers to support non- communicable disease management.

Across Massachusetts, we’re accompanying health departments, community organizations, and health care providers to expand community input into decision-making and improve health outcomes and equity.

In collaboration with the Newark Department of Health and Community Wellness, we’re helping to transform primary care across the city by expanding access to community-based health and social services.

We are accompanying government and community partners as they integrate community health workers into the statewide health system to improve health outcomes and equity in access to care across North Carolina.
